it was /log.php

and it was removed because, as dastrike said, the forums were "de-g-ified" and so that was removed as well

READ AND WEEP — As of a few weeks back, apparently due to a de-g-ification of the forums, there hasn't been a ban log available. So there is no way you can find out yourselves who banned you, and noone can tell you why unless you ask the person in question who banned you, as no reason data is preserved during the ban operation, the ban form doesn't even have a reason field anymore
